Until I know the encounter like the back of my hand, we'll use and follow this write-up generally for our Onyxia attempts.
Read it, learn it, etc. Tell all your buddies to come here and read it/learnit/etc. The less time I have to spend explaining things, the more time we have to learn and conquer the encounter.

I have gone on more than a few failed Unity and PuG's.
Just some random thoughts
One thing people seem to have a hard time with in phase 1 is controled DPS. It only takes a split second of lost aggro for her to turn and flame an entire side of the cave. My suggestion is for the casters to conserve mana and just use wands, hunters use auto shot, everybody else use auto attack. No special abilities. Getting her to fly should not be a race.
In phase 2 its just the opposite. You want to get her out of the air ASAP. After the initial whelps have been killed nuke her big time, full debuffs everything.
I was only in one PuG where we got her to phase 3. Its nuts, we died very soon after she landed.

hmm, i found that link horribly long, lol. IF you guys dont succed this week, i'll see if i can nab you guys a couple spots thursday for the AiD Onyxia run at 7EST, or maybe take a movie if someone can tell me how to do it :P
Generally, the casters try to get onto the walls on eitehr side of onyxia, even groups go to the right and odd groups to the left. The MT keeps onyxia in the middle facing away from the entrance. Some of our hunters like to stay behind onyxia and use their range at max distance. And of course the rogues and warriors would be right next to her fighting. It is ok for casters to cast their spells, HOWEVER if you crit ever you must stop for like 10 secs. I usually let loose a frost bolt and count to 5 and then cast another one.
Make sure the mages are ready for when onyxia takes flight (phase 2). the warriors and mages should go to the place where the eggs are and kill the whelps asap, warriors taunting, mages aoe. People should stay as spread out as possible to avoid a deep breath, there is a mod that tells you where you should go when she is in the air and when she will have a deep breath. Once the first round of whelps are dead, MAX DPS on Onyxia, all dots possible, however stop dots at 40% just before she lands. The mages and warriors must keep a eye on the eggs, every now and then they pop a few whelps.
Just before Onyxia is about the land, i suggest everyone pop a GFP pot and casters should try to get up onto the wall ASAP to avoid lava. In this phase the whole instance goes in combat so you cant keep a resser ooc. Onyxia fears people quit alot and you have to make sure you are away from the entrance to the eggs, otherwise you may get feared into them. By staying up on the wall, you basically just spin in circles. Just keep steady dps and watch your crits. Anyoine who crits should stop and wait a few secs before they continue dps.
Just listen to when the MT calls for assist and watch your crits. At the last 4% or so, everyone once again goes all out MAX DPS and nuke the hell out of her :P Anyway, i think that just about covers it... If anyone knows something i left out just let me know. All this is coming from a mage point of view so i dont know if the other classes do something totally different that what i precieve.

One other small tid bit.
When standing in the mouth of her lair waiting for the MT to run in. All 39 other people must run in right on his heals. If we do not, a warder will respawn and come into her lair with us. It assures a wipe trust me.
